Leone Foundation, Inc. makes financial grants without regard to race, color, creed, religion, ethnic origin, or sexual orientation.

Grant Criteria -

• At the time of application, the child must be between the ages of 3 - 25.  Proof of age is required.

• The child must be seriously or terminally ill, physically challenged, or handicapped.  We will require a letter from your family physician.

• Leone Foundation's sole purpose is to provide financial assistance to families that are struggling with the financial strains of a major childhood illness.  For this reason, families with adjusted gross income at or above $135,000 cannot be considered.  We will require a copy of page 1 of your most recently filed federal tax return.
